AM Ground System Information Collection Form

With some knowledge to the site in question, this form should take about 10-15 minutes to prepare and
submit. Depending on the current load of the 'net, the from should arrive in our mailbox anywhere from 5
seconds to 1 day after you press SUBMIT. If you have not received a call back from us within 24 hours,
your email may be floating around in the ether somewhere... Lost forever. PLEASE call our toll free
number if you have not received confirmation of receipt within 24 hours.

PLEASE NOTE:
Submitting this form implies absolutely NO obligation to you.

This form is meant to make both of our jobs easier and save time.  Please be as specific as
possible but if an answer isn't known we can work it out later.  If some
aspect of your project defies written word, or would take too long to write out, please
fill out what you can, submit the form and call to discuss the rest.  The answers provided on this form
will be used to compile a preliminary quote for this project.

Description (if needed)

The following section is for Directional Arrays ONLY:
Unknown DA specifics
Will radials be shortened and bonded at intersection points?
OR overlapped?
If adding a tower and ONLY constructing new ground field for it, will it be necessary to locate ends of existing radials and bond them to new system?

If further information on any aspect of project is available or needed please provide it below

Thanks for taking the time to complete this questionnaire. We believe that a few
minutes spent in understanding each situation can save hours of work and lots of
